Social Worker-Adolescent Intensive Outpatient-Intake/Discharge Coordinator

Upstate Medical University is looking for a Social Worker-Adolescent Intensive Outpatient-Intake/Discharge Coordinator to join their dynamic team of expert professionals.

The Intake/Discharge Social Work Coordinator provides comprehensive psychosocial care and support to the diverse outpatient population of the Adolescent Intensive Outpatient Program (AIOP) in the Outpatient Psychiatry Clinic. Duties will include intake/discharge coordination by screening patient calls and determining the needs of the patient; providing comprehensive psychosocial consultation through assessment, counseling, intervention, and referral services to meet the psychosocial needs of patients and families; interact with insurance companies, interface with other internal and external agencies regarding referrals and discharges; coordinating discharge planning activities for patients requiring services or facility placement post completion of the program; documenting all interventions. Percentage of time is dedicated to maintaining a patient case load as a co-leader of groups or with individual patients. Participates in departmental meetings, in-services, education, and quality improvement activities; and performing other relevant duties as assigned to include coverage for other social workers and back-up responsibilities.

 Job#73315 MSW from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.
